Transcript Requests

To receive transcripts please download and fill out the form below.  There is a $3.00 processing fee for each transcript payable by cash, check or online.

Transcript Policy:
1. Fees - $3.00 processing fee for each transcript payable by cash or check.
2. Transcripts are usually processed within ten (10) working days of request except at the end of the semester when more time is required.
3. Available transcript rush for two-business day with an additional fee of $10.00.
4. If paying online, please attach your receipt with form.
